Required documents for issuing medical records·image copies

1. If consent can be received from the patient

  • Grounds : Article 21 of the Medical Service Act, Article 13-3 Paragraph 3 of the Enforcement Rules
  • Consent Form and Power of Attorney must include the patient's own name and written signature (identifiable by others) ※ Typed signature, sign, seal, thumbprint not allowed

2. If consent cannot be received from the patient

  • According to Article 21 of the Medical Service Act and Article 13-3 Paragraph 3 of its Enforcement Rules, authority to request issuance of copies can be delegated to a third party by relatives with authority to request copy issuance if the patient's consent cannot be received.
    * If the relatives (spouse of patient, direct descendant, direct ascendant of spouse) are missing, the authority for request is assigned to the brothers·sisters
    (documents confirming that there are no relatives, confirmation form for browsing medical records or copy issuance).
  • If the relatives of the patient are designated as a delegate, additional documents such as Power of Attorney for Medical record browsing and Copy Issuance, ID of delegate, etc. are required.

  • Documents that can prove the fact of missing patient such as Resident Registration Certificate, copy of Declaration of Missing Person from court, etc.
If patient is incapable of decision making
  • Copy of ID card of person requesting record browsing or issuance of copy
  • Copy of judgment on commencement of limited guardianship or adult guardianship from court or diagnosis certificate from a specialized psychiatrist
  • Documents based on patient to confirm the relationship
  • ID : ID including name, date of birth, and photo issued by a public organization
  • Documents for relationship confirmation : Family Relation Certificate issued within the past 6 months (patient-oriented or requester-oriented)

Cost of issue

  • Medical records : 1,000 won per page up to 5 pages, 100 won per page for over 6 pages (e.g. 5 pages: 5,000 won, 6 pages: 5,100 won)
  • Images : 10,000 won per CD, 20,000 won per DVD
